At Revitalize Implant Center, we continuously strive to be considerate and respectful of our patient’s time. On that note, we ask that you print, complete, and return your New Patient forms no less than 48 business hours before your scheduled appointment, so that we may provide you an excellent experience. If you do not have access to a printer, we will have these forms for you when you arrive. We do ask to arrive 15-20 minutes early if you need to fill out of the forms in our office. In some cases, if we have not received your forms, we regrettably may be required to reschedule your appointment to a later date, in order that our other patient’s scheduled time will not be impacted.
